DID YOU KNOW?
Many hospitals and insurance companies are now requiring that physicians obtain
their loss histories every year for their renewal. This can be accomplished
by calling your previous malpractice carriers and asking them to forward you
a “loss history statement”. Many will send them to you within a
week or two. The added bonus of this practice is that you save time and can
make a more seamless transition if you elect to switch malpractice carriers,
or start working at a new facility. If you have questions on how to obtain your

BRIGHT IDEA
Many physicians now keep a “credentials file” on themselves, so
when their reapplications come due, it’s simple and easy to complete because
they have everything in one place! Things to include in your credentials file
are:
- Copies of your CME’s
- Documentation of health items (i.e. yearly TB, Rubella, etc.)
- Copies of Education Certificates (i.e. Medical Diploma, Residency Certificate)
- Copies of Certifications (ACLS, BLS, NRP, ATLS, PALS, etc.)
- State Licensures, Controlled Substance, DEA license
- Certificates of Insurance and Loss History Statements
Just think how easy it would be when it’s time to finish a reapplication
or request for update licensure, when everything is in one place! If you have
